What do you apply with this form?

Use this form to apply for:

• Assessment of the manning proposal and first issue of a Minimum Safe Manning Document (MSMD).
• Renewal of an MSMD.
• Amendment of a manning proposal or MSMD.

For whom is this form intended?

This form is for the scheepsbeheerder (managing owner), who is responsible for exploitation of the vessel. According to the Wet bemanning zeeschepen (seagoing vessels crew Act, in Dutch), the managing owner may be:

1. The owner of the vessel, operating from the registered office in the Netherlands.
2. The organisation to which the owner has transferred responsibility for the operation of the vessel. This company must be established in the Netherlands.
3. The bareboat charterer at the registered office in the Netherlands. The bareboat charterer cannot transfer responsibility for the operation of the vessel.

What is de cost of this application?

A fee applies to this application. You can find an overview of the costs in the Regeling tarieven transportsectoren (Transportation Sectors Tariffs Scheme, in Dutch).

Which enclosures are needed?

For an initial application or a major amendment, you must upload the following attachments:

• The completed manning proposal.
• Shipboard working arrangements (for continuous sailing).
• Proof of UMS (for example, a classification certificate).
• Signed confirmation by the ship master that the proposal is feasible.
• Muster lists.
• General arrangement.
• Work instructions and risk analysis for hatch cover gantry (if applicable).
• Documentation demonstrating that the managing owner and the vessel meet the requirements for "Shore Support" (if applicable).
• Short description of the vessel’s operations.

How does your application work?

When your application is complete, including all required attachments, the ILT will process it within the legal timeframe.
