What do you apply for with this form?

You can use this form for:

• Application for assessment of manning proposal and first issuance of Minimum Safe Manning Document;
• Renewal of Minimum Safe Manning document;
• Changes to manning proposal or Minimum Safe Manning Document;
• Change of ship manager.

For whom is this form intended?

This form is intended for the responsible party in charge of the crew on a seagoing vessel. The Wet zeevarenden (Seafarers Act, only in Dutch) describes the following 3 options:

1. The owner at the registered office in the Netherlands;
2. The organisation or ship manager in the Netherlands to whom the owner has transferred responsibility for operating the vessel. The owner selects domicile at the ship manager's address in the Netherlands.;
3. The bareboat charterer at the registered office in the Netherlands. The bareboat charterer cannot transfer responsibility for the operation of the vessel.

What is the cost of this application?

This application is subject to a fee. You can find the current fee in the Regeling tarieven transportsectoren (Transportation Sectors Tariffs Scheme, only in Dutch).

What happens with your application?

Applications are processed in order of receipt. After submitting your application, you receive a confirmation email with an ILT reference number. Please keep this number for your records and use it in all relevant correspondence with ILT. You will automatically receive a message once your application is processed. You will either receive a decision or a request for further information.

Please note: ILT will not process incomplete forms. This includes missing information or attachments. You will receive a message to update your application within a set time frame.
