What do you report with this form?

With this form, you can report:

• Your suspicion of a violation of Directive 2014/90/EU by a manufacturer, importer, distributor or conformiteitsbeoordelingsinstantie, CBI (conformity assessment body).
• Your discovery of a defective or non-compliant product, bearing a wheelmark covered by Directive 2014/90/EU.

The said directive sets out the requirements to be met by marine equipment. The directive has been incorporated into the Wet scheepsuitrusting (Marine Equipment Act) in the Netherlands.

• Flag state inspectors

Which enclosures are needed?

The ILT will process your report without supporting evidence. However, if you have additional information or evidence, please send any photographs or documents as attachments to your report.

What happens with your report?

The ILT assesses your report and then decides what actions are required. If your report does not give cause for an inspection you will not receive any further notification regarding its handling.
