What do you apply for with this form?
You can use this form to apply for a change of competent authority for your medical records. The SOLI transfer request can only be done if you want to transfer your medical records to the Netherlands. In case you want to transfer your medical records from the Netherlands to another competent authority, please contact the local authorities to which you want to transfer your licence or medical records.

This application is subject to a fee. The fee for transferring your medical records to the Netherlands can be found in the Regeling tarieven transportsectoren (Transportation Sectors Tariffs Scheme only in Dutch).

Application process
After receiving your application, a confirmation of receipt will be sent to you by email. You will receive a request for payment for processing your application. The ILT will only consider your application if it has been submitted correctly and paid for. Incomplete or incorrectly completed forms cannot be processed by the ILT. This also applies to applications without all the required attachments.
