What do you report with this form?

This form is for reporting or asking questions relating to electronic equipment. It specifically concerns the following laws and regulations:

• 2011/65/EU – RoHS, Prohibition of hazardous substances in electrical and electronic equipment;
• 2009/125/EG – Ecodesign, Ecological design of energy-related products.

The report or question may be about a product of your own or a product of a third party.

For whom is this form intended?

For organisations or individuals who have a question about the mentioned legislation or wish to make a (legally required) report.

Which attachments are needed?

Depending on the type of notification, the following attachment(s) may or may not be required:

• A photo or scan of the product, markings or other features;
• Other documentation of the product, such as an EU Declaration of Conformity (DoC) and test reports.

What happens to your notification?

The ILT will deal with your notification and then decide what action is required.
