With this form you can submit an amendment to your merchant shipping certificate or request a duplicate certificate.

Who is this form for?

For ship managers and owners of recreational craft holding ILT certificates for merchant ships.

Is there a fee for this application?

This application is subject to a fee. For more information, see Article 3.21(2)(a) of the Regeling tarieven transportsectoren (Transportation Sectors Tariffs Scheme, only available in Dutch).

How does your application work?

Once you have submitted your application, you will receive an automatic receipt notification. Has your application form been submitted in full, including all the appropriate attachments? Then the ILT will process the application. You will be notified about this. When processing your application, the ILT adheres to the statutory decision periods.

Please note: The ILT does not process incomplete or incorrectly completed forms. This also applies if you fail to send the requested attachments. You will then receive a request to submit the additional information within a certain period of time.
