What can you use this form for?

With this form, you report a health and safety accident on board of a commercial sea going vessel (merchant shipping and fishing industry).

Please note: You must also report urgent incidents to the ILT immediately by telephone. You can call 0031 (0)88-489 00 00. This telephone number is available 24 hours a day.

Citizens and organisations (municipalities and companies).

What attachments do you need to add?

To support your report, you can add photos, reports, a captain's statement or an official report.

What happens to your report?

Applications are processed in order of receipt. After submitting your application, you receive a confirmation email with an ILT reference number. Please keep this number for your records and use it in all relevant correspondence with ILT.

You will automatically receive a message once your application is processed. You will either receive a decision or a request for further information.
