The ILT registers the data from your report and uses it for supervision. During an inspection, an inspector of the ILT may contact you about the report.

Please complete this form in English. Forms submitted in other languages wil not be processed.
You may save the report at any time (see the option in the top right of the screen) which allows you to reopen and complete the form at your convenience.

After completely filling in and sending in the form, you will receive an email which is your confirmation. Included in the email is a PDF copy of what you submitted. The registration number of your report is also mentioned.
