This form is for reporting or asking questions relating to electronic equipment. It specifically concerns the following laws and regulations:
- 2011/65/EU – RoHS, Prohibition of hazardous substances in electrical and electronic equipment;
- 2009/125/EG – Ecodesign, Ecological design of energy-related products.
The report or question may be about a product of your own or a product of a third party.

For organisations or individuals who have a question about the mentioned legislation or wish to make a (legally required) report.
Which attachments are needed?
Depending on the type of notification, the following attachment(s) may or may not be required:
- A photo or scan of the product, markings or other features;
- Other documentation of the product, such as an EU Declaration of Conformity (DoC) and test reports.
