With this form you report a dangerous goods accident or a dangerous goods incident.
Dangerous goods accident:
An occurrence associated with and related to the transport of dangerous goods by air which results in fatal or serious injury to a person or major property or environmental damage.
Example: leakage of an unidentified substance.
Dangerous goods incident:
An occurrence, other than a dangerous goods accident, associated with and related to the transport of dangerous goods by air, not necessarily occurring on board of an aircraft, which results in injury to a person, property or environmental damage, fire, breakage, spillage, leakage of fluid or radiation or other evidence that the integrity of the packaging has not been maintained.
Any occurrence relating to the transport of dangerous goods which seriously jeopardizes the aircraft or its occupants is also deemed to be a dangerous goods incident.
Examples: incorrect labeling, untested package, incorrect documentation.

What happens with your report?
After you have sent in your report, you must keep the package for a maximum of 24 hours. The inspectorate will contact you. If you have not been contacted within 24 hours, you may proceed with the shipment.
All reports will be analyzed periodically in order to recognize trends and developments concerning the safety of transport of dangerous goods by air.
