With this form you apply for a certificate where you prove that you are insured as an owner of a seagoing vessel or fishing boat in case of calamity.

Costs apply to processing the application. You can find the recent costs on the website of the Human Environment and Transport Inspectorate (Inspectie Leefomgeving en Transport, ILT).
For every requested liability certificate you add a transcript of the agreement to provide financial security. This is the so-called ‘Blue Card’.
After the ILT receives your application the inspectorate will check the nationality of the ship. Furthermore, the ILT also checks the information on the ‘Blue Cards’. This includes information of the insurer and the dates of the insured period. You will receive an invoice after issuing your certificate.
