What is the purpose of this form?
With this form you can submit a request to modify a notification after consent from the Human Environment and Transport Inspectorate (ILT). With this form you can request the following changes:
- Adding a carrier which was not mentioned in the notification;
- Changing the border crossing in the Netherlands. Do you want to change a border crossing that does not take place in the Netherlands? Send the change per e-mail to the ILT;
- Increasing the number of shipments. This can be done in case the notifier has used the total intended number of shipments as listed on the notification, but the transport period and the total intended quantity has not been exceded;
- Adding a packaging type;
- Adding a UN-code;
- Modifying the means of transport;
- Company name modification;
- Legal entity or legal form modification.
Who is this form intended for?
The notifier or an authorised representative. You can only modify a notification if the Human Environment and Transport Inspectorate has given her consent on the notification. The notifier is required to get approval for the change from all relevant autorities.
What is the cost of this modification?
There is no charge associated with this request for modification.
How does the modification process work?
The notifier must seek written consent from all relevant authorities for the changes to the notification. Have you received written consent from all authorities regarding the change? Then you can use the amended notification.
