What is the purpose of this form?

With this form you can submit a request to modify a notification after consent from the Human Environment and Transport Inspectorate (ILT). With this form you can request the following changes:

  • Adding a carrier which was not mentioned in the notification;
  • Changing the border crossing in the Netherlands. Do you want to change a border crossing that does not take place in the Netherlands? Send the change per e-mail to the ILT;
  • Increasing the number of shipments. This can be done in case the notifier has used the total intended number of shipments as listed on the notification, but the transport period and the total intended quantity has not been exceded;
  • Adding a packaging type;
  • Adding a UN-code;
  • Modifying the means of transport;
  • Company name modification;
  • Legal entity or legal form modification.

Notifier

This form is for the notifier or authorised representative acting on behalf of the notifier. Use this form to apply for changes to waste shipment notifications that were submitted before 21 May 2026 and have already had a positive decision issued by the Human Environment and Transport Inspectorate (ILT). You must obtain approval from all relevant competent authorities for any changes to an active notification.


If your notification was processed through DIWASS, you must also submit your change request through DIWASS.

How does the modification process work?

The notifier must seek written consent from all relevant authorities for the changes to the notification. Have you received written consent from all authorities regarding the change? Then you can use the amended notification.
